Leading paediatrician, Dr Hermione Lyall of London told the British HIV Association (BHIVA) conference last week that although risk of HIV transmission when breast feeding was very low when viral load was supressed, there was not yet enough evidence to recommend “undetectable = untransmittable” (U=U). Dr Lyall recommended taking a harm reduction approach for women who wanted to breastfeed. She agreed that women would make healthy choices if they had access to support, education and empowerment. She stated that women who wish to breastfeed should be highly adherent to treatment and should minimise the duration of breast feeding with close follow up from their health team.
